Health Equity

The All Hearts Collaborative (AHC) is a health equity initiative led by the HCMA in partnership with the Preventive Cardiovascular Nurses Association (PCNA), BRICK Education Network, and other community stakeholders.


The AHC brings together community groups, houses of worship, and healthcare providers to educate individuals about heart disease. Its focus is on increasing awareness and promoting the timely diagnosis of conditions such as HCM and cardiac amyloidosis — conditions that are often underdiagnosed in underserved communities. All Hearts Collaborative uses community-based outreach to raise awareness of genetic heart diseases and encourages individuals to speak with their physicians about their family's heart history. It also focuses on helping healthcare providers better identify at-risk patients and make timely referrals to specialty cardiac care. For patients who require advanced care, the initiative facilitates referral pathways and offers travel support when needed.
